Stoetzel's Planet Chiropractic (IL) reported to HHS OCR on 2014-03-25 a Theft affecting approximately 1,000 individuals. An unauthorized individual broke into the facility and stole a laptop containing ePHI, including names, credit card numbers, bank account numbers, treatment information, and x-ray images. Breached information located on Laptop and Other Portable Electronic Device. The CE notified HHS, affected individuals, and Illinois media. Post-breach, the CE filed a police report, relocated facilities, installed a security alarm, and enhanced Privacy and Security Rule policies. OCR obtained assurances of corrective action.
