Elite Imaging (FL, Healthcare Provider) reported to HHS on 2016-01-04 a Theft affecting 1,457 individuals. A sign-in log book containing patients' full names and procedure names was stolen from the offices and later returned anonymously with a letter. Breached information was on Paper/Films. The CE filed a police report, notified HHS, affected individuals, and the media, updated policies and procedures, and re-trained staff. OCR obtained assurances of corrective actions.
