Bosch Choice Welfare Benefit Plan (MI, Health Plan) reported to HHS OCR on 2025-10-31 a Hacking/IT Incident affecting 55,000 individuals. A vendor of the covered entity's business associate experienced a cybersecurity incident exposing PHI stored on a Network Server. PHI involved included names, Social Security numbers, dates of birth, claims and health insurance information, and diagnoses/conditions. The CE notified HHS; the BA notified affected covered entities and individuals. Additional technical safeguards were implemented in response.
