Huntridge Family Clinic Foundation (NV) reported to HHS OCR on 2019-06-18 that a board member removed a box containing paper PHI of approximately 2,000 individuals. PHI elements included names, DOBs, addresses, driver's license numbers, Social Security numbers, claims and financial information, and diagnoses/treatment data. After investigation, OCR determined the incident did not constitute a breach of PHI.
