Lebanon Cardiology Associates, PC reported to HHS on November 14, 2016, a data breach affecting 537 individuals. The breach was caused by a rogue employee of a business associate, Ambucor Health Solutions, who downloaded protected health information onto two portable thumb drives. The drives, which have been recovered, contained patient names, contact information, diagnoses, medications, dates of birth, and Social Security numbers. Individuals were notified on June 27, 2016.
