Clarendon Health and Wellness Center (SC) reported to HHS on 2022-06-08 a Theft/insider incident affecting 3,297 individuals. An employee impermissibly downloaded PHI from the entity's electronic health record system. Breached information was located on Desktop Computer, Electronic Medical Record, Laptop, and Other Portable Electronic Device. PHI involved included names, dates of birth, addresses, Social Security numbers, diagnoses, medications, claims information, and other treatment information. The CE notified HHS, affected individuals, and the media, transitioned to a new EHR provider, strengthened security safeguards, and retrained its workforce.
