Lee Memorial Health System (FL) reported to HHS on 2015-09-07 an Unauthorized Access/Disclosure affecting 1,508 individuals. The CE erroneously sent letters to ~1,600 patients containing incorrect patient names due to an administrative error. PHI involved included names, physicians' names and specialties. Breached information was located on Paper/Films. The CE notified HHS, affected individuals, and the media, and implemented corrective actions including new data request procedures and workforce retraining. OCR obtained assurances of remediation.
