Bay Oral Surgery & Implant Center (WI) reported to HHS on 2024-04-26 a Hacking/IT Incident (email phishing attack) affecting 13,055 individuals. An employee was the subject of a phishing attack, exposing PHI located in Email systems. Compromised data included names, dates of birth, addresses, email addresses, phone numbers, Social Security and driver's license numbers, health insurance and financial information, diagnoses, medication, and other treatment information. The CE notified HHS, affected individuals, and the media, and implemented additional safeguards and staff retraining.
