Spartanburg Regional Healthcare System (SC) reported to HHS on 2011-05-27 a Theft affecting 400,000 individuals. Three unencrypted desktop computers and one unencrypted laptop were stolen from an IT employee's vehicle during an unauthorized home stop while transporting equipment. PHI of 402,647 patients was exposed, including names, addresses, dates of birth, and Social Security numbers. Breached information located on Desktop Computer. OCR obtained assurances of corrective actions including full-disk encryption, policy updates, and employee sanctions.
