Henry Ford Hospital (part of Henry Ford Health System) reported to HHS OCR on 2010-11-15 a Theft incident affecting 3,700 individuals. On September 24, 2010, a laptop was stolen from an unlocked office while an employee attended a meeting. The breached PHI included clinical and demographic information (health_basic, identity_basic) stored on a Laptop. The CE sanctioned the employee, deployed encryption licenses, and implemented an encrypted flash-drive program. OCR consolidated this into an existing CE investigation and obtained corrective-action assurances.
