PHI involved included names, addresses, dates of birth, treating physicians' names, and health screening results. Breached information located on an Other Portable Electronic Device. OCR investigated, reviewing HIPAA policies, mobile device training, and risk analysis. The CE subsequently implemented encryption and enhanced its mobile device management program.
