Scurry County Hospital District dba Cogdell Memorial Hospital reported that it experienced a ransomware incident that affected the protected health information (PHI) of 86,981 individuals. The PHI involved included names, addresses, dates of birth, Social Security numbers, medical records numbers, and medical treatment information. The CE notified HHS, affected individuals, the media, and posted substitute notice on its website. In response to the breach, the CE implemented additional technical and security safeguards. OCR provided technical assistance to the CE.
