Ward A. Morris, DDS reported to HHS on 2010-08-11 a Theft affecting 2698 individuals. Breached information located on Desktop Computer. A computer server containing ePHI (names, addresses, DOB, SSN, medical info) was stolen during an office burglary. The server was password-protected but not encrypted. The CE provided breach notification to HHS, affected individuals, and the media, and posted substitute notice. Following the breach, the CE encrypted all ePHI on computer workstations and servers, improved physical safeguards, and retrained employees.
