Breached ePHI resided on a network server and included names, SSNs, dates of birth, addresses, driver's license numbers, diagnoses, lab results, medications, health insurance information, and claims/financial data. The CE implemented additional administrative, technical, and security safeguards and retrained all workforce members. OCR obtained assurances of corrective action.
