Breached information was on Paper/Films and included names, addresses, dates of birth, driver's license numbers, and Social Security numbers. The covered entity and BA retrained staff, revised policies, sanctioned employees, and provided free credit monitoring. OCR completed its investigation and obtained assurances of corrective actions.
