Titus Regional Medical Center reported to HHS on 2012-05-26 the loss of an EMS laptop containing PHI of 5,840 patients. The laptop, which was encrypted, is thought to have fallen off a vehicle fender. If open at the time of loss, names, SSNs, addresses, and dates of birth may have been accessible. A software glitch had permitted excess data storage on the device. OCR investigated; settings were corrected and staff was retrained.
