ARcare

The covered entity, ARcare, reported a malware attack affecting the protected health information (PHI) of 345,353 patients. The compromised data included names, addresses, dates of birth, Social Security numbers, and various treatment information. The breach occurred on a Network Server. In response, ARcare notified HHS and affected individuals, provided complimentary credit monitoring, and implemented additional security safeguards.
