Lancaster General Medical Group (PA) reported to HHS on 2013-03-04 a Theft affecting 527 individuals. A spreadsheet containing PHI — including names and dates of birth — was stolen from one of the CE's locations (Paper/Films). The CE notified police, HHS, the media, and affected individuals, offered identity protection services, and attempted to retrieve the PHI. Following OCR's investigation, the CE reviewed its policies to prevent recurrence.
