Beaumont Health (Michigan) reported to HHS on 2020-01-24 an Unauthorized Access/Disclosure affecting 1,182 individuals. An employee impermissibly accessed PHI stored in Electronic Medical Records, including names, addresses, dates of birth, Social Security numbers, medical record numbers, diagnoses/conditions, and other treatment information. The employee was sanctioned, staff retrained, and additional administrative and technical safeguards implemented. OCR obtained assurances that corrective actions were completed.
