Emergency Room Associates dba Emergency Medicine Associates (AZ) reported to HHS on 2016-05-19 a Theft affecting 1,067 individuals. Five months' worth of hospital labels containing PHI (names, birthdates, ages, sex, treatment facilities) were stolen from a locked briefcase inside a workforce member physician's car parked offsite. The CE notified HHS, affected individuals, and media; filed a law enforcement report; retrained the physician; and, following OCR investigation, conducted a full risk assessment and updated policies and procedures.
