Advanced Fertility Center of Chicago (IL) reported to HHS on 2016-12-01 a Hacking/IT Incident affecting 19,000 individuals. An employee was the victim of an email phishing scheme, exposing PHI including names, addresses, dates of birth, driver's license numbers, Social Security numbers, diagnoses/conditions, lab results, medications, and other treatment information. Breached information was located on Electronic Medical Records and Network Servers. The CE notified HHS, affected individuals, and the media, and implemented additional safeguards and workforce retraining.
