ABQ HealthPartners (NM) reported to HHS OCR on 2013-02-17 a laptop theft affecting approximately 778 patients. The stolen laptop contained ePHI including names, dates of birth, age, sex, referring physicians' names, and raw numeric test data. Following the breach, the CE encrypted ePHI on all laptops and tablets and cooperated with an OCR investigation, providing their IT Security Policy demonstrating HIPAA Security Rule and HITECH Act compliance. Breached information was located on a Laptop.
