Breached information (Paper/Films) included names, addresses, birthdates, phone numbers, Social Security numbers, medical diagnoses/treatments, and insurance information. The CE notified individuals and media, offered free credit monitoring, terminated the storage vendor relationship, and moved records to a new BA under a HIPAA agreement. OCR obtained documented assurances of corrective actions.
