KidsPeace (PA) reported to HHS OCR on 2016-09-19 a Loss affecting 1,456 individuals. A box of documents from the medical records department was discovered missing; it is believed a custodian accidentally discarded it in the trash after it was left near a wastepaper basket. Breached PHI included names, dates of birth, medical record and patient account numbers, and service dates. The CE retrained staff, restricted custodian access to medical records, and OCR reviewed the CE's risk analysis to ensure Security Rule compliance.
