Sheltering Arms Physical Rehabilitation Centers (VA) reported to HHS on 2020-09-16 a Hacking/IT Incident affecting 683 individuals. The covered entity's business associate experienced a ransomware attack affecting ePHI stored on a Network Server. Exposed data included names, dates of birth, marital status, phone numbers, email and mailing addresses, and donor and treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice.
