Aging Disability and Transit Services of Rockingham County (NC), a healthcare provider, reported to HHS OCR on 2019-05-15 a ransomware attack on a network server that affected ePHI of 14,000 individuals. Data involved included names, addresses, phone numbers, dates of birth, Social Security numbers, health insurance information, medical diagnoses, and treatment information. The entity notified HHS, affected individuals, media, and posted substitute notice; implemented additional technical safeguards. OCR provided technical assistance and obtained assurances of corrective actions.
