Centennial Bank, including its division Happy State Bank

Centennial Bank (including its division Happy State Bank) based in Conway, Arkansas, a financial services entity reported a data breach to the Texas Attorney General. The breach was discovered on 2026-08-17 and reported on 2026-09-17. 2,050 Texas residents were affected. 8,227 individuals affected in total. Types of information involved: Name of individual;Address;Social Security Number Information;Driver’s License number;Government-issued ID number (e.g. passport, state ID card);Financial Information (e.g. account number, credit or debit card number);Health Insurance Information;Other;Date of Birth. Consumers were notified via U.S. Mail.

Centennial Bank notified Massachusetts residents that a third-party service provider, Fidelity Information Services (FIS), inadvertently sent a file containing customer personal information (including names, addresses, dates of birth, account numbers, and Social Security numbers) to an unintended recipient at another financial institution on August 7, 2026. The Bank was notified by FIS on August 17, 2026. The unintended recipient confirmed deletion of the file. The Bank and FIS took immediate steps to contain the incident, including recalling the email and terminating access. FIS retrained the associate and implemented enhanced validation procedures. The Bank offered 24 months of complimentary identity monitoring services.
