Hackers accessed the network and installed ransomware. PHI exposed included names, dates of birth, SSNs, driver's license numbers, diagnoses, lab results, medications, treatment info, and health insurance claims. Breached info located on a Network Server. The CE took its server offline, rebuilt the network, and added security safeguards. OCR provided technical assistance on risk analysis.
