Silicon Valley Eyecare Optometry and Contact Lenses

Silicon Valley Eyecare Optometry and Contact Lenses reported to HHS on 2010-05-13 a Theft affecting approximately 40,000 individuals. A computer network server (and a television) were stolen from the covered entity. The server contained ePHI including demographic information, social security numbers, diagnoses, and insurance information. Following an OCR investigation, the entity provided its risk analysis, risk management plan, security training program, and safeguard policies and procedures.
