Seattle - King County Department of Public Health

Public Health — Seattle & King County (WA) reported to HHS on 2013-05-07 an Improper Disposal breach affecting up to 750 individuals. PHI — including treatment/medical condition information and potentially SSNs for five individuals — was inadvertently placed in regular recycling rather than secure disposal. Paper/Films were the breached medium. The CE notified HHS, media, and ~2,300 individuals, updated disposal policies, and was required by OCR to retrain all staff on revised procedures.
