North Texas Medical Center (NTMC) notified the New Hampshire Attorney General of a data security incident discovered on February 26, 2018. NTMC inadvertently provided a spreadsheet containing one New Hampshire resident's PII (name, SSN, DOB, admission/discharge dates, department) in response to a Texas Public Information Act request. NTMC contacted the recipient, who destroyed the data. NTMC offered 12 months of identity protection services to the affected resident.
