The covered entity's business associate experienced a ransomware attack impacting ePHI stored in Electronic Medical Records. Exposed data included names, Social Security numbers, dates of birth, addresses, diagnoses, prescription information, and other treatment information. The CE notified HHS, affected individuals, and the media, and provided complimentary credit monitoring. Security safeguards were subsequently strengthened.
