On October 4, 2013, an unencrypted laptop was stolen from a clinic containing ePHI including names, medical record numbers, account numbers, Social Security numbers, dates of birth, diagnoses, immunizations, and insurance information. OCR opened an investigation and obtained assurances that corrective actions—including full disk encryption, staff retraining, and workforce sanctions—were implemented.
