Pendleton School District 16R (PSD) notified the Nebraska Attorney General of a data security incident involving its third-party administrator, Carruth Compliance Consulting. Unauthorized access to personal information occurred on or about December 21, 2024. The incident affected one Nebraska resident, exposing names, Social Security numbers, driver’s license numbers, financial account information, and medical billing data. PSD notified the FBI, engaged third-party investigators, and sent notification letters on February 28, 2025, offering 12 months of credit monitoring and identity protection services.
