Orlantino Dyoco, M.D. (CA) reported to HHS OCR on 2015-08-03 a physical theft affecting approximately 9,000 individuals. The provider's office was burglarized; a laptop, desktop computer, and backup data were stolen. PHI involved included names, addresses, dates of birth, some Social Security numbers, and claims information. The CE notified HHS, affected individuals, and media. Following the incident, the CE encrypted its computers, strengthened physical safeguards, and moved backups to an off-site encrypted server. OCR's investigation led to a new risk analysis and risk management plan.

On June 2, 2015, the office of Orlantino Dyoco, M.D. was burglarized, resulting in the theft of several computers containing patient information. Affected data included names, addresses, birth dates, telephone numbers, insurance numbers, treatment codes, and billing information. The incident was reported to the Fresno Police Department. Remediation measures included adding encryption to computer systems and providing security training to staff.
