Laborers Funds Administrative Office of Northern California, Inc. reported a data breach on February 17, 2016, involving defective IRS Form 1095-B documents mailed to participants. Due to a computer error, personal information (names, SSNs, health plan coverage) of some individuals was included in documents sent to other participants. The Fund notified the AG, media, and HHS, provided one year of credit monitoring via AllClear ID, and retrained staff.
