Washington Township Health Care District

Washington Township Health Care District notified individuals that an unauthorized individual may have gained access to a computer used to maintain library identification cards on October 8, 2015. The computer contained a database with names, addresses, and driver's license numbers. No Social Security numbers or health information were involved. The District retained a forensic firm and is offering credit monitoring.
