PROMESA (The Puerto Rican Organization to Motivate, Enlighten, and Serve Addicts, Inc.), a New York healthcare provider, reported to HHS on 2020-07-16 a Hacking/IT Incident affecting 3,129 individuals. An employee was the victim of an email phishing attack exposing ePHI via Email. Compromised data included names, addresses, dates of birth, SSNs, driver's license numbers, financial information, passport numbers, diagnoses, and other treatment information. The CE notified HHS, affected individuals, and the media, and responded by strengthening administrative and technical safeguards and retraining staff.
