Painters District Council No. 30 Health and Welfare Fund (IL) reported to HHS OCR on 2021-10-25 a Hacking/IT Incident affecting 2,157 individuals. Employees were victims of an email phishing scheme compromising PHI including names, dates of birth, diagnoses, prescription information, and other treatment information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Additional administrative and technical safeguards were implemented. OCR provided HIPAA Security Rule technical assistance. Breached information located on Email.
