Alive Hospice

Alive Hospice reported that on July 3, 2019, an employee fell victim to an email phishing scheme, resulting in a breach of electronic protected health information (ePHI) for 608 individuals. The compromised data included names, dates of birth, Social Security numbers, health insurance, and treatment information. In response, the entity notified all affected parties, offered one year of complimentary credit monitoring, implemented additional technical safeguards, and retrained staff on identifying fraudulent emails.

Alive Hospice (TN) reported to HHS on 2018-07-13 a Hacking/IT Incident (email phishing scheme) affecting 1,868 individuals (description references 7,858 ePHI records). ePHI exposed included names, SSNs, dates of birth, diagnoses, clinical information, health insurance information, medications, and other treatment information. Breached information located in Email. The CE notified HHS, affected individuals, and the media, offered credit monitoring, and implemented additional safeguards and staff retraining.
