First Mid Bank & Trust, NA reported a production error at vendor HC3 on November 11, 2025, where customer account information (last four digits of account numbers or full account numbers via check images) was inadvertently included in another customer's printed statement. No unauthorized network access occurred. HC3 is remailing corrected statements and offering 24 months of complimentary identity monitoring through Kroll.

The business associate (BA), HC3, reported that an employee mailed the protected health information (PHI) of 1,105 individuals to the wrong recipients. The PHI involved included names, financial information, medications, health insurance information, and other treatment information. The BA notified HHS and the affected individuals. In response to the breach the BA implemented additional administrative, technical, and security safeguards to better protect PHI.
