Jefferson County Health Center notified the Vermont Attorney General of a data breach involving unauthorized access to patient records. The incident involved the compromise of valid credentials, leading to the exposure of personally identifiable information (PII) and protected health information (PHI). The incident status is contained.

Jefferson County Health Center (Fairfield, IA) reported to HHS OCR on 2023-07-28 a ransomware attack affecting 53,827 individuals. Breached information was located on a Network Server and included names, dates of birth, Social Security numbers, claims information, diagnoses, medications, and other treatment information. The CE notified HHS, affected individuals, and media, provided substitute notice, offered complimentary credit monitoring, and implemented additional administrative, technical, and security safeguards.

Jefferson County Hospital, Waurika, Oklahoma, is a 25-bed critical access facility providing medical services to residents of Jefferson County and surrounding communities. 1.1 TB from the medical facility: medical records, test results, and personal information of employees and patients. Accounting and financial information is abundant. This data will be uploaded during upcoming summer release.

PHI on network servers included names, SSNs, dates of birth, addresses, driver's license numbers, diagnoses, lab results, medications, claims, and financial information. The CE notified HHS, individuals, and media; published substitute notice; offered free credit monitoring; and implemented additional administrative/technical safeguards with staff retraining.
