Breached information located on an Other Portable Electronic Device. PHI included names, birthdates, diagnoses/conditions, and other treatment information. The CE notified HHS, individuals, and media, and offered identity theft protection. Following the breach, all BlackBerry devices were encrypted/password-protected, and the encryption policy was revised following OCR investigation.
