Reliant Rehabilitation reported to HHS on 2021-03-22 a Hacking/IT Incident affecting 614 individuals. Breached information located on Email. A workforce member was the victim of an email phishing attack that affected ePHI including names, DOB, diagnoses, treatment info, health insurance info, and SSNs. The BA retrained workforce and strengthened administrative safeguards.

Multi-factor authentication was implemented as a remediation measure.
