The Energy Cooperative Group Benefits Plan

The Energy Cooperative Group Benefits Plan (OH), a Health Plan, reported to HHS OCR on 2022-04-21 a ransomware attack affecting the PHI of 875 individuals. Breached information was located on a Network Server. PHI involved included names, dates of birth, Social Security numbers, and health insurance information. The CE notified HHS, affected individuals, the media, and provided substitute notice. Additional administrative and technical safeguards were implemented in response.
