Compassionate Care Hospice of Central Louisiana, LLC

Compassionate Care Hospice of Central Louisiana, LLC reported to HHS on 2014-09-26 a Theft affecting 707 individuals. Ten encrypted laptops and one external hard drive containing ePHI were stolen. Breached information included patient names, ages, admission/discharge dates, locations, medication classes, addresses, phone numbers, and dates of death. The CE remotely wiped stolen laptops, performed a device inventory, and implemented security improvements including encryption and a risk management plan. OCR reviewed the CE's risk analysis and remediation evidence.
