The breach involved electronic protected health information (ePHI) from a network server, including names, dates of birth, addresses, Social Security numbers, diagnoses, and health insurance information. In response, the center offered complimentary credit monitoring, formalized security policies, and implemented additional technical safeguards.

St. Peter’s Surgery and Endoscopy Center notified patients of unauthorized server access on January 8, 2018. Patient data including names, DOBs, Medicare info (SSNs), and medical codes were potentially exposed. No evidence of misuse found, but investigation was inconclusive. Center offered one year of Experian IdentityWorks monitoring.
