PHI involved included clinical and diagnostic information. Breached information located on a Laptop. The CE notified HHS, affected individuals, and media. OCR reviewed the CE's HIPAA Privacy and Security Rules compliance. Following the breach, the CE accelerated laptop encryption and risk management plan implementation.
