The CE's business associate, InfoCrossing Inc., mailed Missouri Medicaid participants' enrollment correspondence to incorrect addresses from October 16, 2011 to June 7, 2013. Exposed data included names, dates of birth, Medicaid account numbers, counties, phone numbers, and last four digits of SSNs (~1,546 individuals per description). Breached information located on Paper/Films. Remediation included deletion of mailing addresses from system, workforce training, and OCR-documented corrective actions.
