Plumbers Local 8 Health & Welfare Fund (MO, Health Plan) reported to HHS on 2023-08-02 a Hacking/IT Incident affecting 4,790 individuals. An employee was the subject of an email phishing scheme compromising PHI via Email. Exposed data included names, birthdates, addresses, email addresses, phone numbers, claims and financial information, diagnoses/conditions, health insurance information, and other treatment information. The CE notified HHS, individuals, media, and posted substitute notice on its website, then implemented additional administrative and technical safeguards.

Plumbers Local 8 Health & Welfare Fund notified Montana residents of unauthorized access to an employee email account between May 16 and July 29, 2022. The incident involved a limited amount of PHI. The Fund engaged outside cybersecurity professionals, contained the threat, and offered one year of Experian IdentityWorks to affected individuals.

PLUMBERS LOCAL 8 HEALTH AND WELFARE FUND filed a data-breach notice with the Illinois Attorney General during 2023 (case 23-518). The register records the breach as discovered on May 16, 2023. Illinois does not publish the number of people affected — 815 ILCS 530/10 permits the Attorney General to publish only the entity name, the types of personal information and the date range.

On January 23, 2017, the Local 693 Plumbers & Pipefitters Health & Welfare Fund experienced a physical theft where an external computer hard drive was stolen from their office. The breach, reported to HHS on March 13, 2017, affected 1,291 individuals and exposed their names, addresses, phone numbers, and Social Security numbers. In response, the entity implemented new security safeguards and provided a call center for those affected.
